Overview
A disease in which the facial nerve (a nerve that produces facial expressions and secretes saliva) is impaired due to illness or injury, causing symptoms such as difficulty opening and closing of the eyelids and inability to close the mouth. It is divided into two types: central facial nerve paralysis and peripheral facial nerve paralysis. The former has symptoms mainly around the cheek and mouth, and the latter has symptoms all over the face, including eyes and forehead. The main cause of the former is that the brain itself, which transmits information to the facial nerve, is impaired by stroke or stroke. The latter occurs when facial nerves are compressed by fractures and trauma and information transmission is interrupted along the way.In addition, paralysis can occur when the facial nerve is invaded by a virus, which is called Bell palsy or Hunt syndrome.cause
There are three main types of symptoms of peripheral facial nerve paralysis. The first is a sudden paralysis of ′My face doesn′t move when I wake up in the morning′ and ′My face bends when I notice it.′Most of them are caused by the infection of the facial nerve, which is called ′Bell paralysis′ or ′Hunt syndrome.′Second, paralysis occurs after surgery or injury, which is caused by neurosurgery, otolaryngology surgery, fractures of the temporal bone through which the facial nerve passes, and damage to the facial nerve due to deep wounds on the face. The third is a case of slow paralysis, in which facial nerve paralysis occurs as a result of slow impairment of facial nerve due to a special nerve or vascular disease.symptom
Symptoms include not being able to smile well, not being able to close your mouth, and your face looks asymmetrical. Also, the eyes may become abnormally dry or the forehead may not be wrinkled because the eyelids cannot be opened and closed properly.Facial nerves also convey taste information and control the secretion of tears, which can cause abnormal taste and reduce the amount of tears. In addition, in the case of Bell′s palsy or Hunt′s syndrome, the eyelid is automatically closed when the mouth is moved, which is sometimes called an abnormal joint movement (an exercise in which other parts of the body move together regardless of the will of the person). There may be a feeling of a foreign substance in the mouth, or the paralyzed ear may become hypersensitive.examination and diagnosis
In addition to confirming symptoms such as ′I can′t laugh′ and ′Food spills out of my mouth,′ you can also check for paralysis by moving various parts of the face, such as blinking and extending the corners of my mouth outward.Visually inspect the patient to see if there are any characteristic symptoms, such as facial distortion. It will also examine whether the taste and amount of tears are normal.Furthermore, blood tests for checking the cause of paralysis and the possibility of viral infection, MRI tests for photographing the facial nerve, and electromyogram tests of the facial nerve for determining the degree of paralysis are performed.treatment
Facial nerve paralysis caused by surgery or fractured temporal bone is often not expected to recover naturally. However, if plastic surgery is performed to connect the sublingual and occlusal nerves to the facial nerves, it is highly likely that the facial muscles will be able to regain their movements. If the facial nerve is damaged by a deep wound on the face, the damaged facial nerve can be reconstructed. In the case of Bell′s palsy and Hunt′s syndrome, steroid therapy to remove swelling of the facial nerve and antiviral drugs to suppress the growth of the virus are generally performed.By the treatment, more than half of the patients return to their original facial conditions, but there is a possibility that the facial muscle movement is insufficient. In such a case, rehabilitation of facial muscles is performed to reduce aftereffects, treatment for restoring facial movement in plastic surgery, and reconstruction for balancing the right and left sides of the face to return to a state close to the onset.precautions after prevention/treatment
After a while, even if the paralysis is improved through treatment, the muscle strength of each part that was unable to move due to paralysis is reduced, so the facial muscles may not be able to move as before. In this case, a combination of rehabilitation and treatment is required to restore function.Thermotherapy, massage and pronunciation training will be provided.
